What will I need to make an appointment?
- Prescription from referring physician (if applicable)
- Referral from primary care physician (if applicable)
- Proof of insurance
- Valid photo identification
- Worker Compensation- Name of adjuster, claim number, and adjuster/ case manager’s phone number
- Motor vehicle accident- Name of claims adjuster, claim number, and claim adjusters phone number
Most patients will need a prescription form a referring physician. Some insurance plans such as BlueCross FEP do not require a prescription from a physician. In this case you can contact our office directly to be scheduled. If you have an HMO, you will need a referral from your primary care physician (PCP). Please have your PCP indicate the number of session that you are to be seen.
Note: Patients with MDIPA, Optimum Choice can have their PCP order 8 sessions before further authorization from the insurance company is required. BlueChoice- Your PCP can write for 3 visits on the referral before more authorization is required from the insurance company.

What can I expect on my first appointment?
Please arrive 15 minutes early for your first visit, in order to complete all necessary paperwork. If applicable, a co-payment or co-insurance is collected. Next, your physical therapist will perform a musculoskeletal evaluation and develop a treatment plan, which will be discussed with you. Treatment may begin on the first session. You will be instructed on the recommend treatment frequency and be able to schedule for your next appointment(s).
What should I wear to my appointments?
Dress comfortably. Wear loose fitting clothing (i.e. t-shirt, shorts/sweatpants) that will allow the treatment area to be exposed.
How long are the appointments?
Your first appointment will generally be longer than your follow-up appointments due to the paper work and the evaluation. Expect the first appointment to last 60 to 75 minutes. Follow-up treatments usually last no longer than 45-60 min.

Do you work with people who have been in a motor vehicle accident?
Yes, we provide treatment to patients that have suffered from whiplash, strains or sprains. All we will need is your car insurance information. We do not bill the “other persons” insurance company. We will need the following information:
- Name of claims adjuster
- Claim adjusters phone number
- Claim number
- Date of accident
Do you work with people who are under workman’s compensation?
Yes, we will need the following information to help you:
- Name of case manager/ claims adjuster
- Case manager/claim adjusters phone number
- Claim number
- Date of injury
Where are you located?
Sports Pro Physical Therapy offers services in three conveniently located offices in Prince George’s County, Maryland. Our Bowie/ Glenn Dale office is located on the corner of Route 193 – Enterprise Road and Route 450 – Annapolis Road in the Fairwood Office park.
The Upper Marlboro Office is located in the Melwood Professional Center on Marlboro Pike (near the intersection of Route 4- Pennsylvania Avenue and Woodyard Road). We are 2 miles east of beltway 495 exit 11A.
The Ft. Washington Location is location at 930 E. Swan Creek Road, Ft. Washington, MD 20744
